Photo provided

After receiving her bachelor's in social work from Rutgers University, McNally went to work in child protective services, ultimately deciding it was not for her. She went back to school, earning her bachelor's in nursing and master's in nursing education from Western Carolina University. After a year working in a hospital, she realized her love of public health, which led to her applying to George Mason’s brand-new PhD program in public health. McNally successfully defended her dissertation in April.

Throughout her studies, McNally continued to work full time as a public health school nurse for the Fairfax County Health Department as part of the School Health program, which provides schools with school health aides and public health nurses. Her current position as a school health nurse in Fairfax County has been instrumental to PhD goals.

McNally’s primary research during her PhD was looking at ways to improve HPV vaccine compliance, particularly among seventh graders. In fact, it was this goal that led her to getting her PhD in the first place:

“I really wanted to jump right in and do an intervention. But then I had to take a step back when I realized that we didn’t have the research to support developing one intervention over another. And so that’s where it started. [It] was just looking at a way to empower school nurses in particular,” said McNally.

As for what she plans to do now that she’s completed her degree, McNally still isn’t sure.

“I love my job,” she said. “It’s an amazing job. I work with amazing people so I would love to stay in school health.”

However, she also would like to work with schools and the health department to support school health nurses because the one thing she has heard over and over again is that school nurses are overburdened.

McNally is also very passionate about mobile health clinics. She says in the years she has been working in schools in Northern Virginia, she has never seen a mobile health clinic on a registration day or during an open house that offered the HPV vaccine and other immunizations required by Virginia state law for students in public schools.

As she looks toward Commencement this May, McNally said she couldn’t have reached her goals without George Mason and her mentors. McNally’s family, mentors, and well-wishers from the College of Public Health were in attendance as she defended her dissertation.

“Dr. [Amira] Roess [professor of Global Health and Epidemiology] has been my faculty mentor since day one and she’s been really amazing, really supportive. Guiding when guiding was needed but also letting me grow on my own,” McNally said.

She also took inspiration from her mentor Ali Weinstein, a professor in the Department of Global and Community Health, and how well she balanced the research aspect with the people aspect.

“We definitely need numbers and rates, but we also need to understand why people choose to vaccinate or choose not to vaccinate. I do feel like it definitely prepared me for my future goals," McNally said.

“I’m excited to see how Kim will continue to impact the fields of school nursing and public health,” said Roess. “She is a terrific role model for our current and future doctoral students.”

Adds Weinstein: “Her work is impactful and meaningful because of the combination of practical knowledge that highlighted a gap in public health practice.”

<a Her area of expertise is the mental and physical health of postnatal mothers of color.

“There have been research studies on prenatal mothers, but not much data is available for postnatal mothers,” said Vardhan, who is working on a master of public health degree in the College of Public Health. “I look at the challenges [Black and African mothers] face—discrimination and other barriers—and what education models can be provided to them for their optimum health and mental peace.”

Vardhan, who is also a mother to two small children, said the education models created for the postnatal mothers will include how they can take care of themselves after delivery, what herbal remedies can be used in addition to the treatment if they have any medical problems, and how they can advocate for themselves.

In addition to her graduate studies, Vardhan, who is originally from India and came to Virginia in 2013, works full-time as a clinical research specialist at Inova Health System.

<img src="/sites/g/files/yyqcgq291/files/styles/medium/public/2023-12/processed-bcc64b59-b9c2-4c96-ad96-9fa2f202d3c3-c8d6463b-915c-4f50-ba9e-9be7e3819203.jpeg?itok=k0vg2SKx" width="560" height="373" alt="Archana Vardhan. Photo provided.

For her fellowship, she was assigned to work with a health department in Portland, Oregon, where her role included developing a survey about maternal well-being and designing an education program.

“We built a questionnaire, which had a very good response from [around] 110 mothers. Then we did a qualitative analysis. My role was to design the survey and do the qualitative analysis and build an education model on whatever results I get from the survey,” she explained.

Vardhan also enrolled in the summer Global and Community Health (GCH) course GCH 609: Community Assessment and Partnership with assistant professor Marybeth Mitcham, where the coursework perfectly aligned with the responsibilities of her fellowship, she said.

“This course covers what I consider to be some of the most important factors in conducting any community work,” said Mitcham, “[including] the value of involving community members in every step of community work and using an interdisciplinary strategy to address problems.”

The course explores needs assessments, crafting effective surveys, focus group questions, and more, using examples of actual community work, said Mitcham, who is also director of the online MPH program.

Mitcham aims to incorporate students' interests in the classroom so that they can explore topics that are relevant to them. For GCH 609, Vardhan used her research from her summer fellowship and her work at Inova as the basis for her assignments.

“One of my favorite things about [the college] is that students are supported not just in their education and research, but in exploring potential solutions to problems about which they care,” said Mitcham. “Archana is fabulous, and I can't wait to see the amazing positive changes that will occur to the holistic health of postnatal mothers of color as a result of Archana's drive, passion, and excellence.”

Although her fellowship has ended, Vardhan still does volunteer work with AMCHP and is in constant contact with her site supervisor. AMCHP also has a program called Healthy Birth Initiatives that will benefit from Vardhan’s work.

“In the end, it is the mother who is taking care of the baby, so now after giving birth they can be taken care of too. The biggest takeaway is that postnatal care for Black mothers should be culturally sensitive, inclusive, and tailored to address the unique challenges they face,” said Vardhan.

“By focusing on these key areas, health care providers and policymakers can work toward improving the health and well-being of Black mothers and their babies. Mothers are dealing with a huge crisis in terms of emotional [health], especially mothers of color,” she said.

<div His primary assignment focused on infectious disease research about the mother-to-child transmission of specific bacteria in rural Bangladesh.

“The goal of this study was to examine bacteria levels in expecting mothers, their immediate households, and any animals they had contact with to determine how many samples of bacteria were in each mother before birth, and about six months after birth,” Shelton said.

In a separate survey-based study, Shelton also tracked human-deer interactions across Northern Virginia, Washington, D.C., and Southern Maryland to determine risks of disease spread in urban areas.

“The goal of the deer-human interaction survey is to document where the deer are migrating and gain insight on their geographical placement in urban environments,” said Shelton. “This will help determine the risk of zoonotic disease spread between humans and deer in densely populated areas.”

Both studies necessitated Shelton to have an understanding of infectious disease, animal science, and geography, consistent with the public health practice of One-Health.

Under Roess’s tutelage, Shelton designed surveys, performed literature searches, conducted data analysis, and created figures to visualize data---many of which he is already using in his current role with the CDC.

Versatile communicable and chronic disease research skills

In addition to his work with zoonotic diseases with Roess, Shelton is completing research to address the mental health crisis afflicting public health workers across the globe with Project HOPE, a global nonprofit whose public health work includes improving access to mental health services for all populations.

Shelton recognizes the need to reduce the stigma associated with mental health so health care workers feel they can seek help.

“Prioritizing and normalizing mental health is highly important so the workers can share their concerns and be open to receiving treatment,” he said.

Shelton’s interdisciplinary work with Project HOPE integrates the fields of mental health, public health, sociology, and emergency preparedness and response. He is currently working on a resiliency training program to teach frontline health care workers coping mechanisms for high-stress situations.

“It is an unfortunate reality that health care workers do not often receive the mental health support they need in trying times,” Shelton said. “With Project HOPE, I am working on analyzing and improving a mental health and resilience training program that is implemented to address the mental health crisis for nurses, doctors, and other emergency responders around the world.”

Like his work with Roess, the data analysis and public health program evaluation skills he developed while in Mason’s MPH program have proven to be invaluable.

Mason students pursuing the MPH are instructed in the core public health competencies while also being exposed to the various fields where public health intersects with. Those concentrating in global health, like Shelton, are trained to promote public health worldwide, as well as prevent infections and chronic illnesses.

“I took many classes that exposed me to a variety of sub-fields within the public health sphere,” said Shelton. "Within the MPH global health concentration, courses covered a variety of topics including infectious diseases, human migration patterns, and even biostatistics, but most importantly, how these different areas come together in public health."

<div data-block-plugin-id="inline_block:call_to_action" With Mpala Research Centre, a research center based in central Kenya, Lilak worked on a program to detect, predict, and contain illnesses spread between people and animals, also known as zoonoses.

Lilak credits her courses for laying a strong foundation to work with EcoHealth. The MS in Global Health in the College of Health and Human Services teaches students epidemiology, biostatistics, sociocultural influences on health and behavior, global health systems, communicable and non-communicable diseases of global importance, and migrant health. Graduates are prepared to be leaders in global health-related research.

“I went to Kenya to help implement the water processing protocols,” Lilak said. “For EcoHealth, I focused on the water samples. I conducted lab work which consisted of filtering samples, homogenization, extraction, and eventually DNA sequencing. I was also able to participate in field work and sample collections.”

With guidance from her mentors Yvonne Linton, PhD, Michael von Fricken, PhD, Maureen Kamau, DVM, Dawn Zimmerman, DVM, and Brian Bourke, PhD, Lilak participated in field work and sample collections using advanced technologies. MinIONs, for example, is a device that makes it possible to process DNA information in real-time. With the advent of COVID-19 came a rise in psychiatric illness and the incidence of fatal overdoses. Unfortunately, this also coincided with a shortage of health care providers. Thus, Hiba returned to Mason to become a PMHNP and increase access to compassionate behavioral health care.

“Patient care can be considered an art that requires a scientific foundation and authenticity. Evidence-based practice is required to provide appropriate care, though it takes the unique experiences of practitioners to provide more individualized care,” she said. “Substance use disorder needs support, non-judgmental care, and compassion.”

As an alumna of Mason’s Family Nurse Practitioner (FNP) program, Hiba knew that Mason was the best place to further her education in order to gain additional skills to help those with substance use disorder. Mason’s Psychiatric Mental Health Nurse Practitioner certification prepares graduates to perform a variety of mental health services including medication management. PMHNP’s perform a variety of roles including but not limited to conducting assessments, prescribing medication, and providing psychotherapy

Hands-on learning experiences are an essential part of the program. Hiba recently assisted in launching the Population Health Center’s behavioral health clinic. Alongside her cohort, Hiba learned the theory and practice of advanced practice psychiatric nursing in a variety of practice settings within the health care delivery system. Under the guidance of licensed preceptors, students working toward their PMHNP certification meet patients during their initial evaluation and follow them through their treatment journey. She spotlights Dr. Melanie Yousefi, who has supported her throughout the PMHNP program in both the instructional and clinical settings.

“The faculty are remarkable, very caring, and supportive,” she said. “They have so much experience and education that they share with us, ensuring we are prepared to support our community.”

Removing the Stigma of Substance Use Disorder

Hiba is a strong advocate for shedding light on the realities of substance use disorder as a disease rather than a behavior that requires punishment or isolation. She has seen first-hand how the negative perceptions of substance use disorder have dissuaded people from seeking health care. Hiba wants to educate those whose perceptions have been skewed by misinformation.

“When people feel judged they hide away and go through such a dangerous disease process alone,” said Hiba. “With education and compassion, we can remove that stigma, so those who need help are more encouraged to come forward to get the support they need without societal judgment or fear of criminalization.”

Those who suffer from substance use disorder often experience comorbidities including other psychiatric and medical disorders that can go untreated due to stigmatization. Hiba describes how the societally imposed shame associated with substance use disorder causes those who need compassion and support to combat this illness without help.

“People with substance use disorders are human, just like you and me and should be treated with dignity and respect. Substance use disorder is a chronic disease that necessitates holistic care similar to other diseases like diabetes or hypertension. Those battling with substance use disorder deserve the right to receive care like anyone else and live their best life,” Hiba said.

Hiba also notes the importance of utilizing medically appropriate language when referring to substance use disorders as a piece of de-stigmatization. There is a growing movement among health professionals to avoid perpetuating the negative attitudes associated with terms such as drug user and addict. Using vocabulary such as “a person with a substance use disorder” removes the blame from individuals and reframes substance use as a disorder.

No Such Thing as One Size Fits All

Before starting the PMHNP certification process, Hiba worked in cardiology as a Registered Nurse (RN), in an inner-city hospital. She worked closely with the homeless population and incarcerated individuals, where the prevalence of substance use disorder and cooccurring illnesses are statistically higher. In her career as a RN and FNP, Hiba has always made it a priority to meet her patients’ individualized needs and ensure the patient’s support system is prepared to help them outside of her care. She has interned at various inpatient and outpatient facilities, including the College’s Population Health Center behavioral health clinic.

After completing her time at the behavioral health clinic and earning the title of PMHNP, Hiba is committed to putting her education into practice as well as her compassionate nature.

Providing medication-assisted treatment is one of the ways she plans to assist in the interdisciplinary care necessary for substance use disorder treatment. Hiba also plans on working with the adolescent population, supporting them through a vulnerable developmental time frame.

If you or someone you know is suffering from substance use disorder Mason’s Student Support and Advocacy Center offers resources.

Topics

Topics

population health center

PMHNP

Nursing

Nursing News

Student Spotlight

CHHS

Substance Use

Tue, 12 Jul 2022 21:00:45 +0000 Mary Cunningham 72376 at How Pop Culture Ignited a Career in Social Work /news/2022-06/how-pop-culture-ignited-career-social-work How Pop Culture Ignited a Career in Social Work Mary Cunningham Mon, 06/06/2022 - 08:44

Body

Bachelor of Social Work student Alondra Ortega speaks about how her personal values to help children integrate with her career aspirations.

May 2022 Bachelor of Social Work (BSW) graduate Alondra Ortega’s motivation to pursue a career as a social worker was ignited after watching an episode of the popular crime fiction television show Law and Order: Special Victims Unit.
