This is a friendly reminder that the deadline to submit your verification documents for the 2023-24 award year is quickly approaching.

Please ensure that all required documentation is submitted by August 30, 2024 in Patriot Forms.  

Additionally, we want to remind you of an important update regarding our verification documentation submission process:

• Patriot Forms Discontinuation: Patriot Forms will be discontinued as our method of verification documentation submission.  
• New Submission Platform: All future verification documentation must be submitted through Dynamic Forms.

If you have already started your 2024-25 verification process in Patriot Forms, please be aware that this process must be finalized by September 15, 2024. After this date, if your verification is not completed, you will need to resubmit all your documentation through the Dynamic Forms System.
